Liberty Community ServicesHousingPermanent HousingLiberty Safe Haven is a first-of-its kind facility in Connecticut, providing a specialized permanent residence for individuals in our community who have experienced repeated or extended periods of homelessness. Liberty Safe Haven is built on the foundation that no one is beyond the capacity to have a full life, but first we need the security of a place to call home. Liberty Safe Haven is not a shelter or a temporary refuge. It is a permanent, secure and supportive foundation for people who want to rebuild their lives. Residents come at their own free will, stay as long as they want, and pay rent. Liberty provides intensive support services and access to a variety of treatment methods to help them define their own paths to embracing health and recovery. What we seek is to build the trust of our residents. What we offer is a new beginning. (Capacity: 22 adults)
Supportive Living Program offers seven beds in permanent private and shared apartments for individuals at risk of homelessness and living with HIV/AIDS, plus community space, counseling, supportive housing case management, addiction/recovery supports, independent living skills development, crisis intervention and round-the-clock emergency access to staff. (Capacity: 7 adults)
Independent Living Program provides rental assistance and support services for adults living with HIV/AIDS who prefer private scattered-site housing that includes case management, addiction and recovery supports, living-skills development, landlord advocacy and vocational referral. (Capacity: 25 adults)
Open Door Alliance serves adults who are homeless or near homelessness and living with mental illness, addiction or both (and may also have HIV/AIDS). This multi-agency collaborative program provides rental assistance for 16 scattered-site apartments, and offers crisis intervention, counseling, supportive housing case management, addiction and recovery supports, independent living skills development and vocational referral. Capacity: 16 adults currently, (11 of which are at Liberty Safe Haven)
Next Step Initiative, which is similar to our Open Door Alliance program, provides case management through DMHAS with DSS providing housing certificates. (Capacity: 5 adults)
Transitional HousingTransitional Living Program provides 16 beds in private and shared apartments in two downtown Victorian homes for homeless adults living with HIV/AIDS and requiring a host of complementary services for up to 24 months. TLP offers 24-hour onsite staff support, community space, individual and group counseling, supportive housing case management, addiction and recovery supports, independent living skills development, vocational referral, transportation and crisis intervention. (Capacity: 16 adults)
Day ProgramOpen Tuesday through Friday from 8:30 am to 3:30 pm, the Safe Haven Day Program is a welcoming place for people who are homeless to come during the day. Meals are served daily, and showers and laundry are available, as well as groups and other activities that can help people learn new skills and reach their goals.
Admissions and ResourcesLiberty’s Admissions and Resources Program provides outreach, assessment, education and information to individuals and organizations. Supportive housing programs and critical transition case management services are also offered. Admissions and Resources conducts 2,700 eligibility screenings annually, analyzes trends in the needs of the homeless population and makes extensive referrals to other community organizations.
