NEW CSN Interagency Referral Packet
In an effort to improve the referral process and make way for the CSN’s new data management system, the CSN announces publication of a new interagency referral packet.
What’s new about the referral packet?
- Now available in a writable PDF format and may be completed electronically or by hand.
- The format has been reorganized to accommodate additional helpful information and to correspond to the CSN’s new data management system, Provide Enterprise.
How do I use the new referral packet?
- Use when requesting any CSN Residential, Social Rehabilitation and/or Vocational Services.
Submit by mail or in person to the CSN office at CMHC, Room 144, 34 Park Street, New Haven, CT 06519 or by secure fax to 974-7719.
Where can I find the new referral packet?
When can I start using the new referral packet?
- Effective Monday, February 9, 2009.
- NOTE: The CSN will no longer accept old versions of the packet starting Wednesday, April 1, 2009. Referrals using the old version will be returned for re-submission.
