Supportive Housing ProgramSHP is for clients who are struggling with homelessness, mental illness and co-occurring substance abuse. This federally funded program provides comprehensive services including transitional specialized housing with a structured, dual-diagnosis emphasis; a broad range of vocational supports; and coordinated dual-diagnosis clinical treatment. For referrals to SHP programs, please fill out an Inter-Agency Referral Form and the included SHP Screening Form. SHP Programs: Fellowship Place, "The Fellowship Inn" - Provides engagement into the system, transportation, breakfast, a morning program, and community living skills training. Continuum of Care, Inc. "Frank Street" - Provides specialized transitional housing and supportive services with 24 hour on-site staffing utilizing a therapeutic community model with a dual diagnosis focus. Men only. ALSO-Cornerstone, Inc. "Pendleton House" - Provides specialized, transitional housing and supportive services with a structured, dual diagnosis treatment focus and outreach/engagement component. Staff on duty 6:00 a.m. - 9:00 p.m, 7 days per week. Coed. Columbus House "Sojourner's Place" - Provides transitional housing for women, including supportive services, with 24-hour on-site staffing. |
